Once you have reviewed the instructions to proceed to online forms
ATHLETIC PARTICIPATION DIRECTIONS:

The Heritage Hall Athletic Department would like to thank you for taking part in our process of accepting the required athletic participation packet

1. You can fill out the required physical forms online by clicking on the tab located at the top named "Required Physical Forms." These forms can be filled out online but must be printed and taken with the student to their Doctor's appointment to received actual physician's signature. Once completed these forms must be delivered to Heritage Hall Athletic Administration.

2. To access the required athletic participation forms for Heritage Hall Schools, click on electronic forms tab “Electronic Participation Forms” tab. You will get a drop down list of all required forms.

3. TO BEGIN, click on the form name and fill out the information requested. If you do not know your student ID# NOT REQUIRED leave box blank. If a question is not applicable, please answer question with NA or UNKNOWN.

4. TO SIGN THE DOCUMENT, click inside the signature box and hold your mouse down, this will allow you to create an “electronic signature.” if you make a mistake and need to start over click on the refresh icon next to the signature box. If you are using an iPAD, you may create electronic signature, by holding your finger to the screen and signing or by using a stylus pen and signing.

5. You will receive a confirmation email once the document(s) have been reviewed by a Heritage Hall Athletic Administrator.
